Bureau County
Health & Wellness Clinic

GOALS:

Our goal is to make a difference in the lives of our community by caring for the members of the community who are sick, poor, under served, and at risk.  We do this by:
  1. Providing primary health care to the uninsured, low-income population.
  2. Provide routine diagnostics to enable health care providers to fully diagnose and treat our patient population.
  3. Assist patients in complying with treatment by providing access to medications through an in-house formulary and specific patient assistance programs.
  4. Provide education to patients and the community to give them the tools to make healthier decisions.
  5. Meet emergency clothing, dietary, and monetary needs of our patients through an in-house emergency food shelf, collaboration with a local resale clothing store, and the Bureau County ministerial association, who manages a fund for emergencies.
  6. Continue to identify and evaluate needs in our community, planting new free clinics and establishing satellite clinics where appropriate.
  7. Work closely with area social service and faith-based community agencies to fully utilize all existing resources.
