Health & Wellness Clinic
GOALS:
Our goal is to make a difference in the lives of our community by
caring for the members of the community who are sick, poor,
under served, and at risk. We do this by:
- Providing primary health care to the uninsured, low-income
population.
- Provide routine diagnostics to enable health care providers to
fully diagnose and treat our patient population.
- Assist patients in complying with treatment by providing access
to medications through an in-house formulary and specific patient
assistance programs.
- Provide education to patients and the community to give them the
tools to make healthier decisions.
- Meet emergency clothing, dietary, and monetary needs of our
patients through an in-house emergency food shelf, collaboration with a
local resale clothing store, and the Bureau County ministerial
association, who manages a fund for emergencies.
- Continue to identify and evaluate needs in our community,
planting new free clinics and establishing satellite clinics where
appropriate.
- Work closely with area social service and faith-based community
agencies to fully utilize all existing resources.
